About Tripod Academy
Tripod Academy is an international training platform that brings together independent, accredited Tripod Beta Trainers, Assessors and experienced incident investigators.
All trainers operate through their own organisation and bring a distinct combination of sector experience, cultural perspective, language capability and specialist expertise to the network.
By sharing experience across countries, industries, and professional disciplines, we broaden one another’s perspectives and strengthen how Tripod Beta is taught and applied. This collaboration helps us respond thoughtfully to different organisational contexts while maintaining a shared commitment to methodological quality, professional investigation practice and meaningful organisational learning.
Beyond delivering training, we are practitioners of Learning from Incidents. Our work is centred on Tripod Beta, professional incident investigation, and the organisational learning that should follow.
Across the network, we promote the thoughtful and consistent application of Tripod Beta in alignment with recognised standards.
Trainer and Assessor credentials are independently certified by the Stichting Tripod Foundation.
Tripod Academy is a trade name of Geniozz BV and serves as a specialist platform for Tripod Beta knowledge, training, and professional development.
Learning Events offered through the platform are delivered by accredited trainers who participate in the Tripod Academy network through their respective organisations.
The organisation responsible for delivering each Learning Event is clearly identified during registration and manages the subsequent administration and invoicing.
Methodology expertise shaped by real investigations
Our international network includes accredited Tripod Beta Trainers and Assessors who continue to work in incident investigation, risk analysis and organisational learning across complex and high-risk environments.
Trainer and Assessor credentials are independently certified by the Stichting Tripod Foundation.

Our Mission
We help organisations strengthen their investigation capability, build learning capability and advance the practice of Tripod Beta through learning events, resources and investigator development.
We share a common belief: organisations do not improve simply because investigations are completed. They improve when operational experience is translated into meaningful organisational learning. Every Tripod Beta Learning Event we deliver is designed with that objective in mind.
Strengthened by Different Perspectives
Tripod Academy connects professionals from different countries, industries and areas of expertise. Some bring extensive experience in incident investigation, while others contribute perspectives from risk management, barrier thinking, safety leadership, sustainability or organisational learning.
These differences are valuable. They allow us to challenge familiar assumptions, compare practices across sectors and learn from approaches developed in different operational and cultural environments.
Where appropriate, trainers consult one another, exchange lessons from practice and combine complementary expertise to support a programme or learning event.

Lisbeth Holberg
-
Accredited Tripod Trainer
-
Safety Leadership Specialist
-
Based in Denmark, delivering worldwide
-
Lisbeth participates in the Tripod Academy network through HOLBERGs, her independent learning from incidents, safety leadership and training practice.
Lisbeth supports organisations in strengthening investigation quality, operational learning, and safety leadership. She is an accredited Tripod Beta trainer and independent safety and leadership consultant who helps organisations turn incidents into insight, learning and safer operational performance. Her work is guided by a simple belief: no one goes to work intending to cause an accident. Yet when incidents happen, the person closest to the event is often blamed. Lisbeth helps organisations move beyond this last triggering action and understand the wider system of local and organisational conditions, decisions and dilemmas that shape everyday work – both in the office and at operational sites. Before establishing her consultancy in 2009, Lisbeth spent 9 years in the oil and gas industry, working on QHSE in exploration and production. Since then, she has supported organisations across many industries, particularly offshore wind, oil & gas, construction and power generation. Lisbeth has been a member of the Flying Squad since 2010 – a pool of lead investigators available 24/7 for worldwide deployment following major process disturbances and serious incidents. Drawing on this experience, Lisbeth works closely with leaders and investigation teams to help them answer a question many organisations struggle with: what exactly should leaders do to create safer workplaces? Her focus is always the same – turning incidents into insight, real learning and practical improvements, not just another report on the shelf.

Asyraf Khalil
-
Accredited Tripod Trainer
-
Risk Management & Incident Investigation Specialist
-
Based in Malaysia, delivering across Southeast Asia
-
Asyraf participates in the Tripod Academy network through Cikgu Barrier, his independent risk management, incident investigation and training practice.
Asyraf combines Tripod Beta with practical experience in risk management and process safety environments. He is an accredited Tripod Trainer, accredited BowTie trainer and Risk Management specialist with over a decade of consulting and training experience across Oil & Gas, Maritime, Plantation, Aviation, Utilities, Government, and Higher Education sectors in Malaysia. His work is grounded in a practitioner's frustration: most organisations treat risk assessment as a documentation exercise and incident investigation as a blame-finding exercise. Both produce reports that sit in folders and change nothing. His programmes are designed to break that cycle - giving investigation teams the analytical rigour to trace incidents back to the systemic conditions that shaped them, and giving risk teams the tools to build assessments that reflect how work actually happens. For nearly ten years - from 2014 to 2024 - Asyraf served as Lead Consultant and Trainer at Khalil Trading Services, working directly with clients across Malaysia's most demanding industries. His client work spans major PETRONAS subsidiaries, Sime Darby, MISC Berhad, Tenaga Nasional Berhad (TNB), Dialog, Dayang Enterprise, Bintulu Port, JKKP (DOSH Malaysia), NIOSH Malaysia, Lynas Malaysia, and Sepang Aircraft Engineering, among many others - delivering risk management consulting, BowTie workshops, and Tripod Beta investigations at both operational and board level. In 2020, he was invited to serve as Examiner at the Post Graduate Symposium on Educating Process Safety Culture towards Smart Society at the Faculty of Engineering, Universiti Putra Malaysia. Today, Asyraf leads Cikgu Barrier - Malaysia's specialist training provider for Risk Management and Incident Investigation - where he delivers practitioner-led programmes designed to close the gap between what organisations know about safety and what they actually do about it.
