Tripod Lite
A free incident investigation tool for simpler incidents and near misses
Not every incident requires a full Tripod Beta investigation.

Yet many serious incidents are preceded by smaller incidents, operational deviations or near misses that receive only limited investigation. Looking back, organisations often discover that the warning signs were already there, but because those earlier events were investigated only superficially, valuable learning opportunities were missed.

Tripod Lite was developed to address exactly this challenge.
Based on the principles of Tripod Beta, Tripod Lite provides a practical and structured way to investigate simpler incidents and near misses. It helps organisations move beyond recording what happened and understand how and why an event occurred.
Developed under the responsibility of the Stichting Tripod Foundation and made freely available through the Energy Institute, Tripod Lite enables organisations to strengthen everyday learning from operational events.
Tripod Lite is not intended to replace Tripod Beta. It complements the methodology by providing a practical investigation approach for incidents that do not require the depth of a full Tripod Beta analysis.
A Tripod Academy Perspective
One of the most common misconceptions in incident investigation is that only serious incidents deserve a structured analysis.

Interactive PDF and Excel versions
Tripod Lite is available in two formats.

The interactive PDF contains a visual barrier-based investigation model that guides users step by step through the investigation process. It can be completed electronically or printed and completed by hand.

For organisations requiring greater flexibility, a Microsoft Excel version is also available. This version allows users to customise drop-down menus and record additional barriers and causes.
