Oscar Diederich
Accredited Tripod Assessor and Trainer
Learning From Incidents Specialist
Based in the Netherlands, delivering worldwide
Oscar Diederich is an accredited Tripod Beta Assessor and Trainer with extensive international experience in incident investigation, risk management and organisational learning. Between 1996 and 1999, he was closely involved in the development and automation of safety-related processes, including the Tripod Delta Survey and Tripod DeltaBase. This early involvement provided a strong foundation in systemic safety thinking and analytical methodology. Since 2006, Oscar has worked as a trainer and consultant specialising in Tripod Beta, BowTie risk analysis and safety culture development. His work focuses on strengthening investigation quality, barrier thinking and the practical application of structured analytical methods. He has been a member of the Flying Squad since 2007 - a pool of lead investigators available 24/7 for worldwide deployment following major process disturbances and serious incidents. Through his work as a trainer, consultant and lead investigator, Oscar has gained extensive experience across oil and gas, chemical processing, power generation, maritime, aviation, infrastructure, healthcare and rail. His approach combines methodological rigour with practical applicability, supporting professionals in developing robust, defensible and learning-oriented investigations.

Tatjana Draese
Accredited Tripod Trainer
HSE and Sustainability Specialist
Based in Germany, delivering worldwide
Tatjana is an experienced HSE and sustainability professional with more than 20 years of leadership experience in high-risk industrial environments, primarily within the aluminium rolling industry. Throughout her career, she has held senior leadership roles with a strong focus on incident reduction, operational performance and organisational development. Her work integrates systemic safety thinking with practical implementation in complex production settings. Tatjana supports organisations in strengthening incident investigation practice, improving barrier management and embedding sustainable safety performance. She helps leadership teams understand their current safety maturity and translate strategic objectives into concrete, achievable measures - whether related to regulatory compliance, continuous improvement or long-term cultural development. Her approach combines structured analytical methods with holistic, tailored concepts. She frequently applies the Hearts & Minds framework to support cultural transformation and to align leadership behaviour with safety values. In addition to programme design and delivery, Tatjana works directly alongside operational leaders, coaching them in real-time decision-making and practical application within day-to-day operations. She is also a member of the Flying Squad - a global network of experienced investigators available 24/7 to support organisations following major process disturbances and serious incidents.

Lisbeth Holberg
Accredited Tripod Trainer
Safety Leadership Specialist
Based in Denmark, delivering worldwide
Lisbeth Holberg is an accredited Tripod Beta trainer and independent safety and leadership consultant who helps organisations turn incidents into insight, learning and safer operational performance. Her work is guided by a simple belief: no one goes to work intending to cause an accident. Yet when incidents happen, the person closest to the event is often blamed. Lisbeth helps organisations move beyond this last triggering action and understand the wider system of local and organisational conditions, decisions and dilemmas that shape everyday work – both in the office and at operational sites. Before establishing her consultancy in 2009, Lisbeth spent 9 years in the oil and gas industry, working on QHSE in exploration and production. Since then, she has supported organisations across many industries, particularly offshore wind, oil & gas, construction and power generation. Lisbeth has been a member of the Flying Squad since 2010 – a pool of lead investigators available 24/7 for worldwide deployment following major process disturbances and serious incidents. Drawing on this experience, Lisbeth works closely with leaders and investigation teams to help them answer a question many organisations struggle with: what exactly should leaders do to create safer workplaces? Her focus is always the same – turning incidents into insight, real learning and practical improvements, not just another report on the shelf.

Asyraf Khalil
Accredited Tripod Trainer
Risk Management & Incident Investigation Specialist
Based in Malaysia, delivering across Southeast Asia
Asyraf Khalil is an accredited Tripod Trainer, accredited BowTie trainer and Risk Management specialist with over a decade of consulting and training experience across Oil & Gas, Maritime, Plantation, Aviation, Utilities, Government, and Higher Education sectors in Malaysia. He is certified by the Stichting Tripod Foundation and the Energy Institute, UK to design and deliver Tripod training and award Bronze accreditation. He also holds a Train-the-Trainer certification for BowtieXP, IncidentXP, and Tripod Beta under CGE Risk in Amsterdam. His work is grounded in a practitioner's frustration: most organisations treat risk assessment as a documentation exercise and incident investigation as a blame-finding exercise. Both produce reports that sit in folders and change nothing. His programmes are designed to break that cycle - giving investigation teams the analytical rigour to trace incidents back to the systemic conditions that shaped them, and giving risk teams the tools to build assessments that reflect how work actually happens. For nearly ten years - from 2014 to 2024 - Asyraf served as Lead Consultant and Trainer at Khalil Trading Services, working directly with clients across Malaysia's most demanding industries. His client work spans major PETRONAS subsidiaries, Sime Darby, MISC Berhad, Tenaga Nasional Berhad (TNB), Dialog, Dayang Enterprise, Bintulu Port, JKKP (DOSH Malaysia), NIOSH Malaysia, Lynas Malaysia, and Sepang Aircraft Engineering, among many others - delivering risk management consulting, BowTie workshops, and Tripod Beta investigations at both operational and board level. In 2020, he was invited to serve as Examiner at the Post Graduate Symposium on Educating Process Safety Culture towards Smart Society at the Faculty of Engineering, Universiti Putra Malaysia. Today, Asyraf leads Cikgu Barrier - Malaysia's specialist training provider for Risk Management and Incident Investigation - where he delivers practitioner-led programmes designed to close the gap between what organisations know about safety and what they actually do about it.
